We are the NHS in Staffordshire and Stoke-on-Trent.  

We follow the laws about keeping your answers and information safe.

Our surveys ask questions about you, like how old you are, if you are a man or a woman, and the postcode for the area where you live.

This information will help us to make sure we have heard from lots of different people from different areas. 

Any reports we write from our surveys will not have your name in them.

Any reports we write about the survey will not have your name in them.

You can stop the survey at any time, and you do not have to answer all of the questions.   

All information in the survey will be kept for 5 years from the date the survey closes.
